2gether Care is based in Brisbane and along the Sunshine Coast and focusses on building relationships with many local and trusted NDIS providers. From allied health to educational pathways, your independence is at the top of our list.

We are a support coordination provider who connect you to suitable supports or community programs and link you to available mainstream activities to enhance and fulfil your life.

Not sure if you have support coordination in your plan?
We are very happy to guide you through your NDIS plan to see if this is available and how to get it if it’s not. Support coordination is valuable in many cases for example if you have a limited support network or multiple disabilities.

We take the hard work out of navigating the NDIS by connecting you to the support you need.
